Coordinated Clinical Care Through a Trusted CQC-Registered Partner

‍

When a person’s needs include regulated personal or clinical care, Holding Together Healthcare works alongside a trusted CQC-registered partner rated Good by the Care Quality Commission.

The registered provider leads the assessment, planning and delivery of all regulated care. It remains responsible for clinical oversight, staff supervision, risk management, care records, quality assurance and compliance with CQC requirements.

Holding Together Healthcare acts as the liaison between the case manager and the registered provider. We provide a consistent point of contact, support clear communication and help ensure that relevant updates and agreed actions are shared between both parties.

Where appropriate, we can also attend multidisciplinary team meetings to support communication and coordination. All clinical decisions, regulatory responsibilities and regulated activities remain with the CQC-registered provider.

Our buddy and community support can work alongside the regulated care package, with each organisation’s responsibilities clearly agreed before support begins.

How the Partnership Works

  • Dedicated liaison: Holding Together Healthcare provides a consistent communication link between the case manager and the registered provider.
  • Multidisciplinary working: We can attend MDT meetings and help communicate relevant updates and agreed actions.
  • Clear responsibilities: Each organisation’s role is agreed from the outset, ensuring clear responsibility and accountability.
  • Coordinated support: Our buddy and community support works alongside the regulated care package and the person’s wider goals.
  • Consistent communication: We help reduce communication gaps by keeping the case manager and registered provider appropriately informed as the package develops.
